As part of our commitment to data protection we value your right to privacy. Details shared on your application, including those of the applicant, recipient and medical professional will be held securely and we will never disclose, share, sell or rent any data to a third party, other than to regulated authorities when required to so by law. I, the applicant, by applying for a Care Pack confirm that I have read, understood and accepted the terms and conditions herein. You may withdraw your application at any time by notifying us at care.pack@lmcancertrust.co.uk
You are applying for a Care Pack as the recipient is facing a cancer diagnosis.
You have not made a request to Lasting Memories Cancer Trust Care Pack service previously within the last 24 months.
As part of the application process, you will be required to submit proof of the recipients identity and address ie. driving licence or passport. By agreeing to these terms you are declaring that you have consent from the recipient to share their identity and address. This data will be used in accordance with our privacy policy and GDPR regulations.
We reserve the right to request additional evidence in order to process your application.
Failure to provide all the information required or submit any additional information requested may result in Lasting Memories Cancer Trust being unable to award a care pack to the recipient.
Lasting Memories Cancer Trust will provide a care pack free of charge and will not accept any liability for any loss, damage, injury, cost or claim arising from items used within the care pack.
The contents of the care pack will be based on available stock at the time of postage.
We complete one postage run per month so there may be a short delay in receiving the care pack based on the date of your application.
You will receive a letter confirming the outcome of your request within 14 days and details of any terms of the service that is being provided. You will be asked to sign and confirm that you agree to the terms of receiving the care pack within 14 days of receipt of the letter.
You may be asked to provide feedback to Lasting Memories Cancer Trust to ensure the Charity can review its procedures and processes. We may also ask for a testimonial to be used in marketing material.
We may contact the medical professional to verify details of the recipients medical condition.
By submitting an application you confirm that the information is a true statement of all the recipients medical circumstances to the best of your knowledge and belief.
